Birth, Death & Marriage Request Forms
Please note, if you are looking for a record pre-1920, that is considered a genealogy request. A $10 Research fee is required to conduct the research for any requested genealogy document. Additional fees for certified documents are required to be paid before receiving documents. Please allow 6 to 8 weeks processing time for research of any genealogy documents (there is an additional 2-week delay due to current Election prep). If you have any questions about genealogy, please see the Genealogy information page or contact the Town Clerk's Office directly.
We now offer online ordering for Birth, Death, and Marriage certificates on the UniPay website. If other documents are required, please email the Town Clerk.
The Town Clerk can provide certified copies of all birth, death, and marriage records that are on file with the Town. The Massachusetts State Registry of Vital Statistics has records for all towns and cities in Massachusetts.
The Town of Stoughton will only have the following Vital Statistic Records on file:
- Birth records: Anyone born in Stoughton or whose parents were living in Stoughton at the time of the birth. In some cases, picture identification may be required to obtain a certified copy of the record.
- Marriage records: Any couple who filed their intentions to be married in the Town of Stoughton. Please note that even if the marriage took place in Stoughton, we will not have the record unless the marriage intentions were filed here.
- Death records: Any death within the Town of Stoughton or if the deceased was a resident of the Town of Stoughton at the time of death.

Copies of birth, marriage, or death certificates cost $10 for each copy.
