The Stoughton COA outreach workers are here to help! The staff can make home visits to provide information and refer seniors to the programs and services available to them through the COA and other agencies. If you or someone you know needs assistance or a well being check, please call our outreach coordinator at 781-344-8882. They can provide application assistance for fuel assistance, SNAP benefits, MassHealth and Medicare (SHINE), as well as housing applications
