As a Medicare Certified Home Health Care Agency, we are not just your local Public Health Department, we operate as a business. We deliver Visiting Nurse services, which include Skilled Nursing visits, Physical Therapy visits, Occupational Therapy visits, Medical Social Worker visits, and Home Health aide visits. All of these services are ordered by your Physician, and are provided to you in your home. These are services that are generally required after certain surgeries, hospital stays, or when an individual is debilitated and high risk while living in their home. The Certified services are for people who are not well enough to leave their home, but there are Private Pay services available to anyone interested in Home Care services.

· Assess, manage and evaluate medically necessary interventions during episodic events

· Promote wellness through education, screening and immunization

· Foster independence through family and community support and resources

· Offer comfort, care, support and dignity through terminal illness

· Meet the Public Health Care needs of the Stoughton community

We might ask that you consider asking for the SVNA whenever you or a loved one is hospitalized and being sent home with services. Although we are a non-profit Agency, we are a self-sustaining Enterprise Fund, and quite confidant that we provide the most compassionate care around.
